Restarting firefox will not always restore / reopen windows and tabs

If I close firefox by "kill -9", or power cycling the system, then it manages to reopen windows and tabs without any problem next time.
If I use File -> Exit, there is a good chance that only one window -- the last one to be closed -- will survive and reopen next time. This is not constant -- perhaps 20% of the time -- but it's enough to make me want to use kill -9, which in turn starts talkbalk the next time.
Reproducible: Sometimes
Steps to Reproduce:
1. Have multiple windows with multiple tabs
2. File -> Exit
3. Restart.
Actual Results:
Usually fine.
Occasionally: Only one window survives to reopen.
Expected Results:
Many windows.
Rather than giving me window by window "Do you really want to close 3 tabs?", how about one "Do you really want to close 9 windows and 31 total tabs?".

I get the same behavior. I hadn't thought of using kill -9, it's good to know that's a workaround.
Version 2.0.0.7 here.
Duplicate of Bug 427468?
My guess is that you guys are unknowingly experiencing Bug 398817. New findings in Bug 427468 clarified this.

Can any of you reproduce this issue on Firefox 3.0? IIRC there's been a fix for a Mac specific issue where windows weren't closed all together but window by window (which would lead to the behavior described).
