I have the Tabs option "When I open a new tab, switch to it immediately"
un-selected. I also have "Accept cookies from sites"|"Accept third-party cookies"|"Keep Until: ask me every time" selected.
These two interact very badly. If I middle click on a link to open it (in the background) in another tab and that site wants to do a cookie, then FF 3 switches to that tab and displays the Accept This Cookie dialog. The cookie dialog goes away when I answer it _and_ that tab gets focus and retains it, which is contrary to my settings.
Reproducible: Always

The accept cookies dialog is modal, just like if a site would post an alert(), which automatically grabs focus no matter where you are so this is pretty much intentional.

Except that Firefox has never behaved this way before. It certainly doesn't behave better than it used to, so it's a regression.
