When the accept cookies option is set to "ask every time" and the dialog comes up to ask the user, it stacks on top of itself for the number of cookies from that domain, even if for the first one you click "use my choice for all cookies from this site" it doesn't dismiss the others.  So if a site puts MANY on your computer on first load, you have to click through ALL of them to even get to the site itself.

This has only occurred for me under Firefox 3.* and didn't happen on versions before that.  Unfortunately it only seems to happen on certain domains/pages, but did occur for me on the URL for this bug

Reproducible: Sometimes

Steps to Reproduce:
1.  Configure Firefox to "Ask every time" and to "allow 3rd party cookies" in the security tab in options.  Also ensure the URL linked doesn't already have accept/reject rules defined for the user, or any existing cookies.
2.  Go to the site indicated above, http://www.audioreview.com/reviewscrx.aspx
3.  If the indicated behavior does not occur, click on the "A/V Receivers" link to get to this site: http://www.audioreview.com/cat/amplification/a-v-receivers/PLS_2718crx.aspx
4.  Ensure that the "use my choice for all cookies from this site" is CHECKED and pick any of the 3 options.
Actual Results:  
Many dialogs asking if you should accept/reject cookies should appear directly underneath the one you already clicked which SHOULD have defined the behavior for the entire site.  You can also move the original dialog to see that the subsequent ones are already there (all for the same domain), not that they appear after the first click.

Expected Results:  
Only one dialog for the domain should appear.  If there are subsequent 3rd-party sites they should have their own dialogs, but for any one domain or sub-domain should only have one dialog if "use my choice for all cookies from this site" is checked.

This bug doesn't seem to happen for EVERY site out there that sets multiple cookies, but it NEVER happened in Firefox 2.x, but happens in 3.x under both Windows XP and Vista.  I haven't explicitly tested this under Linux, but the behavior is expected.

The only consistent addons used between the machines that have this issue is Adblock Plus (using the Easylist Subscription), FireGestures (shouldn't affect URLs), and IETab (shouldn't affect this issue).
I have closed downstream Fedora bug https://bugzilla.redhat.com/show_bug.cgi?id=449402 and I can easily reproduce the reproducer with firefox-3.0.1-1
Can confirm (see screenshot) that this bug still exists in 3.0.1 (ubuntu, x86_64) and is a DOS vector.  Fix would be to revert to FF 2.X behavior e.g., only present a single dialog box at a time, applying the response from first dialog to subsequent dialogs.
