I found some shortcuts that are existing more than one time in the settings dialogue. So pressing them will cause either of them being selected. 

Here we go:

'Allgemein':
"Startseite:" + "Download-Manager schließen, wenn alle Downloads abgeschlossen sind" (Alt+S)

"Aktuelle Seite verwenden" + "Alle Dateien in folgendem Ordner abspeichern:" (Alt+A)

'Datenschutz': 
"Private Daten löschen, wenn Firefox beendet wird." + "Daten speichern, die in Formulare und Suchleisten eingegeben werden" (Alt+D)

'Erweitert' / 'Allgemein': 
"Suche bereits beim Eintippen starten" + "Rechtschreibung während der Eingabe überprüfen" (Alt+R).

"Sanften Bildlauf aktivieren" + "Beim Starten überprüfen, ob Firefox Standardbrowser ist" (Alt+S).

Reproducible: Always

Steps to Reproduce:
Go to Extras > Einstellungen.
Actual Results:  
Some shortcuts are duplicates.

Expected Results:  
Fix the shortcuts

On my Linux, I can confirm Alt+D, Alt+R and Alt+S with the latest firefox 3.0.x nightly but not Alt+S and Alt+A.

Yes, this is for Firefox 3.
