On servers that support the getctag extension, if there has been no change to the calendar ctag on refresh() we (properly) don't check the calendar for updates but fail to check the inbox for updates; this can make it difficult for the user to see newly-arrived invitations. Apple is such a server. patch in hand, needs testing
