Menus in OS X respond to cmd/opt-up and cmd/opt-down to jump to the first and last items of said menu, respectively. They behave exactly like the home and end keys, thus.
The menu bar is not affected, since it uses native OS X menus.
Reproducible: Always
Steps to Reproduce:
1. Open a Firefox context menu.
2. Press cmd-up or opt-up, cmd-down or opt-down.
Actual Results:
Nothing happens.
Expected Results:
The selection should jump to the top or bottom of the menu, depending on the arrow key pressed.
The desired behavior can be checked on any native menu.
(There's a chance opt-up/down actually behaves more like page up and page down; I don't have at hand a menu large enough to test it, though. It should have enough options to fill at least two whole pages with options to confirm.)

Just a minor correction: the actual result is that the modifier keys are ignored, and the selection moves up/down only one item.
Also, just to make it clear, the home/end keys already implement this behavior (and they are correct in doing so). So, in principle, it's just a matter of binding those same actions to additional key combinations.

This bug's priority relative to the set of other polish bugs is:
P4 - Polish issue that is rarely encountered, and is easily identifiable.
Obvious when you encounter the bug, but the set of effected users is limited to those who actually hit command-[arrows] in context menus.
