The following is seen.

- Open Firefox
- Have any webpage open
- Press "CTRL + T" or press on the New tab button.
- The URL from the previous in focus webpage (or tab) is copied into the new tab.

Expected: no input in the address bar.

Reproducible: Always

Steps to Reproduce:
1. Open Firefox
2. Open a webpage
3. Press CTRL + T
Actual Results:  
The URL from the previously in focus webpage/tab is seen in the address bar.

Expected Results:  
No input in the address bar

A new tab opens always blank here, so this could be the result of an extension.

Good call!

It's the Skype add-on for Firefox, the one that comes with Skype 3.8.0.108. This particular add-on turns every phone number on a webpage into a "click-to-call" button.

I've since removed it - I wasn't using it anyway - and the problem has disappeared.

Do I need to report this any further? I'd like to make a small positive effect to the Firefox community...

You could report the problem to the extension's author, so he can fix it. Bugs found in add-ons are not valid Firefox bugs. If you would like to help the Firefox community, you could test nightly builds. There is every day a new one at http://ftp.mozilla.org/pub/mozilla.org/firefox/nightly/latest-trunk/
