There is a lot of cleanup that we could do in nsPluginHostImpl.
This does at least the following:
- removes an old workaround for mac flash 6
- stop using the phrase "active plugin" to reference plugin instances, that is confusing because an "active plugin" sounds like a plugin that is not blocklisted, loaded into memory, or something else like that
- remove some dead code
- fixes some spelling
