491660 - Randomly disappearing frequently used address from adaptive query

I've have a few site that i visit using the sqlite shortcuts:
typing "go" in address bar should suggest "www.google.it",
typing "ddl" should suggest "http://ddlfantasy.net/"

Randomly the shortcuts are reset: if I type
"go" it returns "http://mail.google.com/mail/?hl=it&tab=wm",
"ddl" becomes "http://ddlfantasy.net/download/index.php?showforum=110"

yesterday the shortcuts were working fine, but this morning after updating the stylish extension i've encounterd this problem;
I think that this happens every time an extension has been updated.
This is my extension list:

adblock plus,
cooliris,
firebug,
firegesture,
html validator,
IE Tab,
Reload every,
TVU web player,
Stylish,
Weave.

I have this problem since Firefox version 3.0.8

What do you mean by "sqlite shortcuts"?  Bookmarks?  Typing into the location bar?

So, after an irc talk, the problem is that when typing g in the locationbar and choosing Google (, the adaptive query should suggest always google between first results.
Instead on some day the adapative query miss the suggestion, and the Google results slips down between the last results.

Steps should be:
1. visit www.google.com
2. digit "g" in the locationbar and choose www.google.com (many times)
3. visit many other websites
4. digit "g" in the locationbar
EXPECTED:
google is the first choice
ACTUAL
sometimes google is between the latest results

Do you never go to the other pages by typing "go" or "ddl"? Are you clearing history in any way? How many times do you type "go" and select it? Are you exactly typing "go" or "goo"?

looking at the database, gmail has a really high frecency of 66000, google has about 3000.
in inputhistory for "g" there are entries for gmail, gentoo, but there are no entries for google.

Also all history entries to the google homepage before 05/05/2009 are missing.

looking deeper, location bar results are correct comparing them with database values. So the question moves more toward database values correctness, or more to why there are so few visits for pages the should be visited more (weave problem? redirects problems?)

Reporter, are you still seeing this issue with Firefox 3.6.10 or later in safe mode or a fresh profile? If not, please close. These links can help you in your testing.

No reply from reporter, INCOMPLETE. Please retest with Firefox 3.6.12 or later and a new profile (http://support.mozilla.com/kb/Managing+profiles). If you continue to see this issue with the newest firefox and a new profile, then please comment on this bug.
