When I launch Firefox, even when it's the first and only app launched, it is not the front window. It acts like another window is in front of it. Keyboard shortcuts do not work as a result since the window is not focused. I have to Command-Tab or click on the window to make it focus.
I'm running a clean install of Snow Leopard with personal data imported using migration assistant. However I've deleted firefox and the Application Support data and reinstalled the app but problem still exists.

Yes.
However it doesn't do it when I create a new user account and launch Firefox in there.
Have I missed something when I try to remove everything to reinstall?
Macbook/Library/Application Support/mozilla

Are you creating a new firefox profile, or a mac user account? I just need you to create a new FIREFOX profile, using the directions given above.
Yes I tried creating a Firefox profile per your instructions but that did not clear up the problem.
After reading your instructions, I thought about trying to launch firefox on a fresh Mac User account. When I logged into that account and launched firefox, I did not have the focus issue. So it seems to be something specific about my profile.

Do you have Growl installed? That would explain the difference: Growl is active for your normal user account but not the newly created one.
The focus issue is a known problem of Growl on 10.6 (still happens with the current 1.2b3 version).
Yes I do and killing that program fixed the problem.
