Bug 170143 removed four columns from Page Info using the labels formNo (twice), mediaNo and linkNo, but it didn't remove the entities from pageInfo.dtd; bug 379183 then removed the mediaNo entity but the other two are still there.

It looks like bug 463886 had already removed 'formNo':
