If a tab is loaded in the background, the zoom level will not be fitted to the page's width.

There is a patch in bug 557222 that fixes this.

Does the foreground tab need to be zoomed before opening the new tab in the background? Can you add some more context past this if necessary?

No, it doesn't matter whether or how the foreground tab is zoomed. Any time a tab loads completely in the background (i.e. it finishes loading before it is selected for the first time) its zoomLevel will start at 1.0 instead of the usual default behavior.
