Insecure HTTP form submission dialog needs title

Details
Some dialogs, such as the security warning, don't have titles which looks odd
and is probably bad UI. Also, Windows uses the active dialog title in its
ALT+TAB box.
Perhaps you can dump a warning when a window is displayed without a title?

The one I have in mind is the form submission over HTTP warning.
Are we supposed to report each untitled dialog separately?

Ok, that title for that window, following the usual rule of naming a window after
the object which it contains explains, would be `Insecure Form Submission'.

Unless the bug is assigned to Blake Ross, yes that would probably help.
