If you check Accept Images from the Originating Server Only in Preferences|Image, nothing will be shown in 
place of the refused image.
This should not be the case, especially when the image has a hyperlink.

this seems like a good idea to do, and shouldn't be too hard. Confirming, 
not sure if this a layout or prefs or something completely different.

I'm not sure if I understand what you mean with "a place holder". Can you please
explain this to me?

If an images doesn't get loaded, for whatever reason, the alt text should be
displayed inplace of that image, without changing the layout!
Not in a case like this attachment, where size or ALT text is defined. In this
testcase, at least the border is shown, and the link can be clicked. However, if
you set border=0, then there will be nothing shown.
