Multiple address entry in mailing list dialog

Ths bug happens with 54530's fix. Seps to reproduce.
1. Create a mailing list with at least one email address entry.
2. Close Address Book window
3. Reopen Address Book window
4. Edit the mailing list just created, now you see two identcal email address
entries
I should have fix for this bug.
Reasign QA contact to Suresh.
This bug is just a UI bug, the data in the database is stored correctly. I also
found out it seems only happened on the first mailing list created in the
address book. I'll try again.

These are the step I can reproduce every time.
1. create a new address book or delete all entries in personal address book
2. create a mailing list(list1) with one address entry
3. create another mailing list(list2) with the same address entry
4. close address book window
5. reopen address book window, select List1 and click Edit
6. there are duplicate address entries in the list
