When I have Firefox maximized and hide the "Menu Bar", all I see is the orange Firefox button in the corner and no minimize, maximize, or exit buttons in the other corner. When I have the "Menu Bar" hidden, I can see the title bar but I cannot see the Firefox button and there is a small gap between the "Menu Bar" and the title bar. The gap goes away when I am not maximized, but there is still no Firefox button.

Steps to Reproduce:
I don't know that there are steps to reproduce it. It's always like this for me.
Actual Results:  
Nothing, it's just an annoying part of the interface. It doesn't limit functionality, but it makes it harder and more bothersome.

Expected Results:  
I am under the impression it should have a minimize, maximize, and exit button when the "Menu Bar" is hidden and the Firefox button should still be visible when the "Menu Bar" is unhidden.

It still appears with every theme I use, including the incompatible "Default Theme".

it does, but you also have a persona installed, and that is mentioned in the release notes which is a well known issue bug, dupe of bug 574833.
