This is fully repeatable in Ubuntu 10.04 and in 9.10. I have two computers, one is Ubuntu 10.04 (upgraded from 9.10) and other was 9.10 until few days ago and after fresh install of 10.04 the problem still persists.
Screen shot of the state after starting Firefox can be found here

Also it should be noted that localstore.rdf has line
<RDF:Description RDF:about="chrome://browser/content/browser.xul#main-window" width="994" height="714" screenX="284" screenY="24" sizemode="fullscreen"/>
This line is saved when the browser is closed.

Updated to Firefox 3.6.15 (Ubuntu 10.04) and problem persists. Slight improvement is that if after restarting Firefox F11 is pressed and Gnome top applet bar is on auto hide the window controls appear. If the top bar is not on autohide then controls are under the top applet bar.
BTW safe mode Firefox does not behave similarly and does not suffer from this bug. It may be that handling the window is somewhat different when Firefox is started.
