There are still issues with large cache sizes (512MB in my case), Firefox just stops adding new items to the disk cache after some time of working as it should. Most notably all new images show up as size: unknown (not cached) and it barely generates new files in the cache directory, even when visiting sites I know I haven't been visiting before.
With the planned increase on default cache sizes this should probably be looked in to beforehand, I can't give you a quick way to reproduce this state of the cache though as it takes roughly a week of my normal browsing activity to trigger it.

I encountered the same problem.

Here is my status:

It doesn't increase any more thus all new images show up as size: unknown (not cached) like Simon.
Oh. I forgot to report my system.
OS: WinXP
Firefox 4.0b8

And It was updated directly from a firefox 3.6 profile if important.
Looks like the changes to b10 fixed this issue.
