We don't need the component picker, but the rest should be the same, and point bugs to Mozilla Services: Operations. Please use services-infra as the group for "internal" issues and leave that checked by default. It's been six years since I've touched bugzilla templates, so if we can mod itrequest without copying, even better.

Well, it's a different product, and I think the target audience will be end users, so this is the wrong place. Having had four weeks to think about this, I actually want a different form: Username: [ ] Sync Client: [ Firefox 4 ] | Firefox Sync add-on | | Firefox Home | | Other | When did the issue appear: Issue/error description: And that's it. There's probably also a link to services.mozilla.com/status in there, and maybe some branding/messaging from product.
Can we add "security issues" into the work flow? Would like to see a break out for sync security specific issues. So here is the proposed list of questions. We can break this out into another bug if you want too. We should also state that this information is not going to be public so we will need to make sure security boxed is checked. Background Questions: 1. What is the account name that you use for Sync or Firefox home? 2. How many devices do you have syncing? 3. What are the device types? 4. Which device did you first notice this issue on? 5. Do you have all the devices in your control?
Mike and Chris: I'm pretty busy right now, and I'm fairly sure Dave is as well, so one thing that would be a great help to Dave and me in getting this going is to get some bare HTML/JS/CSS with all the functionality and display characteristics you all want. I'm happy to add the backend Bugzilla magic into it, but the UI part of the forms is what takes me forever. There are a couple of examples under http://bzr.mozilla.org/bmo/3.6/files/head:/template/en/custom/bug/create/ that you can use, if you want. The important thing to remember is that you can have as many fields and such as you want, but at the end, there needs to be appropriate JS that combines all those fields into one comment that will be used as the initial description. Again, we've done this already for a few different bug creation forms, so you can get some ideas from the above directory. Let me know if you have any questions.
Actually, if they're getting dumped into the comment section, we can combine those together server-side. See the comment-* files in that directory reed gave for examples of the server-side combination templates. If you're doing things that affect fields other than the comments (like doing magic combinations of multiple fields into the summary or component fields) then you'll need pre-submit js magic for that.
Is this form still needed? If yes, did the mockup for the HTML ever get submitted or do you want us to do that based on some of the other templates we already have? dkl
