(Apologies if this is a duplicate, I had trouble working out the proper names for things.)
I have, on my present Firefox session, got a phantom tab when I rollover the Firefox icon on the taskbar. I can't look at it (probably because it no longer exists), and I can't exit it using the little "x" alongside it. Other tabs are shown fine, and update properly*.
It shows (see URL) a YouTube tab which I exited over an hour ago now, with a plain "page" favicon rather than YouTube's normal favicon.
In terms of reproducibility, I haven't tried restarting Firefox yet. Will do so in a minute.
*Briefly, a few minutes ago, the list only showed the current tab and no others. But that's a separate bug. When normal functionality was restored, the phantom tab was still there.

Restart cleared it; now can't reproduce, alas.
Also, I accidentally set this to file handling when I meant General, apologies.

Still present in beta 6; am going to attach to trunk.

(And when I say "present", I mean with a completely different, much less intensive website. It's definitely a Firefox problem.)
Another thing worth a mention is that in present session (continued from above), they seem to be accumulating. I know have 5 "Phantom tabs" on the go at the same time.
I'm having this happen as well.
Trying to close those phantom tabs from the taskbar preview logs the following errors to the console:

I have sometimes the same problem.
I have now tried to reproduce it without success.
But if I find a way to reproduce it, will report it here.
