In 4.0 beta 5 on OS X 10.6.4, if I go to a page that displays a Flash video (e.g., http://adblockplus.org/en/), every tab has a blank hole where that flash video was displayed.

I have the same issue and it's new to beta 5 @ osx. It happens after the Flash plugin crashes (but not every time it crashes!).
Also, sometimes the Flash player from another tab steals the keyboard focus (I don't know how to call it). It just happened now, I had a youtube video playing on another tab and every time I hit the spacebar on this fpage, the video got paused/resumed.
See the same issue here on OSX 10.6. And its not just in other tabs, for example if you use Piwik web analytics, the flash "white box" scrolls around on the page obscuring text on the page. Other tabs are impacted and the box is "stuck" until you kill the tab. Nothing else seems to remove it.
It seems to be less tied to flash "crashing" per se but the flash plugin exiting in a tab [aka from a refresh or a forward back action]. Once it does (or crashes) it leaves a remnant that requires a tab close.
This is definitely new in B5. Disabling all addons, etc had no impact.

I have a similar problem, probably due to the same bug, flash objects appear in all tabs, the flash app itself not a hole like the one in the screenshot. I'm using Linux and the version 4.0b6 (Mozilla/5.0 (X11; Linux i686 on x86_64; rv:2.0b6) Gecko/20100101 Firefox/4.0b6)

The mac issue should have been fixed in beta6.
This Linux issue with beta6 I would like more info on. Is the issue reproducible or just happens sometimes? Does it happen if you use a current nightly?

tn, which bug fixed the mac problem? I'd like to close this mac-specific bug out and get another one filed on Linux if there's any more information.

Bug 592563 should have fixed the Mac problem in beta5.
If anyone (Victor?) is seeing a similar problem on beta6 or newer please file a new bug and cc me.
