I'm not 100% sure that this is a bug, but: if you go to: http://worldradar.dk/ this site incorporates a reload of the page using: <meta http-equiv="Refresh" content="240; http://worldradar.dk/"> but after the reloads done, the old site is added to the history. If you leave your browser at: http://worldradar.dk/ for a long time, the History is floded with "World Radar" entries. Neither Netscape 4.76 or IE adds the "new" (same) site to the history.
