Testing without addons is one thing and another would be to disable the hardware acceleration in tools/options/advanced/general. A third thing would be to disable TM/JM in about:config or you could do all 3 things together with starting Firefox in the Firefox safemode.

I find this happens frequently when I alt-tab out of a Direct3D application (using 4.0RC1 under WinXPSP3 with nVidia 7600GT). Is it the same for you?

This is worrysome. This could be a bug in the D3D9 backend, but I'd expect more people to be seeing it, I wonder if this is a driver/device combination issue, in that case we could blacklist. (or as far as I can see it -could- still be non-D3D9 related, but it sounds unlikely)

@ annihilannic: can you please share your GPU information? Go to about:support, copy, paste here the Graphics section.

...and meanwhile, we already see that the two reports here are both on Geforce 7 series GPUs.

Apologies, I had forgotten I had upgraded to an 8600GTS:
Graphics
Adapter Description: NVIDIA GeForce 8600 GTS
Vendor ID: 10de
Device ID: 0400
Adapter RAM: Unknown
Adapter Drivers: nv4_disp
Driver Version: 6.14.12.6099
Driver Date: 10-16-2010
Direct2D Enabled: false
DirectWrite Enabled: false (0.0.0.0, font cache n/a)
WebGL Renderer: Google Inc. -- ANGLE -- OpenGL ES 2.0 (ANGLE 0.0.0.541)
GPU Accelerated Windows: 1/1 Direct3D 9
Video memory is 256MB. Is there a Firefox build number somewhere? All I can find is:
Build identifier: Mozilla/5.0 (Windows NT 5.1; rv:2.0) Gecko/20100101 Firefox/4.0
