If you reload a page multiple times, for each time you reloaded it a new entry
is added to the history. This doesn't happen at most sites, but one site it
does occur at is http://www.mozillazine.org This is with Mozilla build
2000-12-08. So far it has been consistently reproducible.

please be more specific. Added to the history where? Where are you looking to see multiple
entries?

Oops, sorry. They're added to the history under the <- button.

uh-oh. I tried on mozillazine.org and repeatedly clicking reload caused an additional entry
to be added to SH every time (seen under the back menu popup). I believe it also replaced
the forward entry as well since the forward button no longer lit up! Repro'd on Mac 2000120809
