I'm running Firefox 4.0.1 with Flash 10.3.181.14 on WinXP SP2 behind an NTLM authenticating proxy server.

Steps to reproduce

1. Load http://demo.swfupload.org/v220/simpledemo/index.php
2. Click Hello and select 5 small images (40-50KB) to upload
3. "Connect to demo.swfupload.org" windows authentication dialog pops up
4. Enter your domain/username and password click OK
5. Second dialog pops up with username and password already prefilled, click OK
6. Browser hangs for ~1 minute then "Adobe plugin crashed" appears on top of the page

I get two crashes reported

When I run Chrome 11.0.696.57 and Flash 10.2.154.27, I get the same two windows authentication dialogs then a hang and then "Plug-in Unresponsive" "Would you like to stop it? Yes|No" chrome dialog box.

@marius - thanks for the STR.  it definitely helps.  i've added them to our internal bug and will make sure it gets another once over.

@marious - we are unable to reproduce the crash.  can you attach screenshots of the dialog boxes so we can make sure we are attempting the steps correctly.

I'm running Firefox 5.0 right now. In step 6, I have to click cancel icon next to file being uploaded then I get a browser hang and shortly the plugin crash. 

I'm also running Symantec antivirus software which I have read is messing with upload progress of SFWUpload widget (the upload progress bar goes from 0% to 100% even though no data is being transferred to the server).

One more thing... I've noticed that when I'm running Fiddler (web debugging proxy) then I get no Windows authentication dialogs and the file upload seems to work OK. I get "Completed" status.

I'm pretty sure my issue is the BlueCoat proxy we're using (BlueCoat SG510). HTTP request gets intercepted and redirected to the proxy. Proxy then responds with 401 and asks for NTLM authentication. After that it sets authentication cookie and redirects the browser back to the URL.
