I had Flash that was embedded as 'wmode' = 'transparent'. I changed it to 'wmode' = 'direct', because IE9 seems to have problems with transparent that slow it to a crawl, and it now most embedded Flash shows simply as a square box.
I can tell that they're still running (both by seeing that the plugin-container process is still there, and that they're requesting the correct files though Firebug).
The Flash was embedding using SWFObject.
Reproducible: Sometimes

1. Embed a Flash using SWFObject, with wmode = direct
2. ?

Note this is the latest version of Flash (11) and it could be that there is conflict between Flash rendering GPU to output, as well as GPU rendering in FF4? I think Flash 11 does this (or tries to) by default if running in direct mode.

From where do you got it and what is the exact version number (open about:plugins)

I am not sure if it is related to this bug or not, but after setting the hidden pref. plugins.force.wmode to "opaque", I see the same behavior (black box instead of Flash content). You can see this with any YouTube video, e.g.,

If plugins.force.wmode is set to opaque, you will hear the audio but the video image is not displayed.

It seems I got this bug or some sort. Running 13.0a2 with the latest Flash installed (11.2 r202) I get a blank spot where the flash video should be. I can still interact with the flash content and even play videos and hear sound. I navigate to this page

and all I see where the flash should be is white.
Add-ons installed are Adblock Plus SPDY indicator and Test Pilot.

Make sure you have the latest drivers (very important) and software (Firefox 16+ / Flash Player 11.5+) installed and try it again please.
A DirectX 9 update can help sometimes:

We're in the process of removing support for plugins (bug 1677160), so I think this bug is irrelevant now.
