Prominently show which plugin was loaded for plugin crashes

Details
In plugin crash/hang reports, it would be really helpful to prominently show what plugin it was that this process was running.
Not sure if Socorro know this in a useful way right now, but it can at least be derived from the stack if we don't have anything better.

The client sends this info. If you look at a plugin report:

You can see:
Process Type plugin Version: Filename: NPSWF32.dll
it's kind of hidden in there. We could definitely make it more visible.
