Steps to reproduce:

In Thunderbird versions 3.1.9+ there was a keyboard accessible Install button in the add-ons manager that easily allowed me to install an extension from a file. Now with the new manager this is totally cut off to the blind. In fact there is an entire add-ons options menu button that is not accessible by the keyboard. There is absolutely no way for me to access this menu. I propose to change it to a simple context menu. That menu as the feature I desperately need.

The add-ons manager utilities menu is indeed out of taborder; or even if that is the tab stop between the tab (on the tab bar) and the add-ons manager search box, I could not open the menu from the keyboard howsoever I tried.
