I had a url query where the "switch to tab" result was below another result. Apparently, this is not supposed to happen.

Me too. For using switch-to-tab as a feature, results should always be on top.
This happens to me too. I am using a 32-bit version of Firefox 5 on 64-bit openSUSE version 11.3 on Linux.

btw, this is supposed to happen, those entries have the same frecency ordering than others
If what you want is to have the list show only open tabs, add a percent sign.
