I had a url query where the "switch to tab" result was below another result. Apparently, this is not supposed to happen.
Me too. For using switch-to-tab as a feature, results should always be on top.
This happens to me too. I am using a 32-bit version of Firefox 5 on 64-bit openSUSE version 11.3 on Linux.
This bug report seems to be in conflict with bug 610720. (See my comments there.)

If what you want is to have the list show only open tabs, add a percent sign. See: http://support.mozilla.com/en-US/kb/Location%20bar%20autocomplete?s=awesome+bar&r=0&as=s#w_changing-results-on-the-fly
