From bug 648668:

The greyed-out icon (rect(32px, 16px, 48px, 0px)) in winstripe's chrome://global/skin/icons/folder-item.png is probably now unused.

If this icon is indeed unused, we should remove it.
MXR tells me this icon is referenced in:
/toolkit/themes/winstripe/global/filepicker.css
/browser/themes/winstripe/browser/places/editBookmarkOverlay.css
/browser/themes/winstripe/browser/places/places.css
/browser/themes/winstripe/browser/preferences/preferences.css
/browser/themes/winstripe/browser/searchbar.css
/browser/themes/winstripe/browser/browser.css

Actually, I was wrong because I just looked at the whole icon. The (32px, 16px, 48px, 0px) part seems unused.
seems 'folder-item.png' doesn't exist anymore.
