"Restore Default Set" doesn't restore buttons placed on external toolbars

If you remove a toolbar button from a primary toolbar (e.g. the Navigation Toolbar in Firefox or the Mail Toolbar in Thunderbird) and place it on an external toolbar, restoring the default button set removes the button from the external toolbar but doesn't add it back to the primary toolbar. Clicking "Restore Default Set" a second time returns everything to normal, though.
Steps to reproduce:
1) Customize the Navigation Toolbar in Firefox
2) Drag the Home button to the Add-on Bar in Firefox
3) Click Restore Default Set
4) The Home button should be on the Navigation Toolbar, but it's on the palette
instead
5) Click Restore Default Set again
6) Everything's back to normal

I'm pretty sure we fixed this in australis - and it's no longer possible to add toolbars yourself, and the add-on bar is gone, so marking this wfm.
