I've attached the test_bug691215.html file in case I happen to move it or remove it before this bug gets looked at. The file is a modified version of the one at content/xml/document/test/test_bug691215.html in mozilla-central (the modifications are to make it standalone and not depend on SimpleTest).

mDocument is null.

At first glance, looks like a duplicate of bug 691215 (which was fixed in Firefox 10).

Can you reproduce the problem in a Firefox 10 beta build?  Or just Firefox 9?

Oh!  You're just using the testcase from bug 691215, but running in a build without that fix.
