Please see bug 157681 comment 85 (first and second paragraph in the response, and also the comment 87).

This test case https://gist.github.com/2387812 seems like it should trigger this case, but it does not for some reason.  Note that the patch in bug 157681 fixes the bug here, but we still need to add a test case to make sure we do not regress this.
