We should do something a little smarter here. People seemed confused about it. Maybe we should introduce the notion of portrait and landscape. Also, we don't update the width/height pair in the menu (only the presets), and we don't show the status of the "rotate" state. This is confusing.

I think a toggle landscape/portrait button is the way. Width and height values already imply the orientation. On my tool I use min and max for my presets. It's the orientation determines if min value should be used for width or height. If we introduce this min and max notion maybe we should go from "320x480" to "320/480" or "320|480" in the preset display label. Then the order shouldn't be a problem (to be verified by users of course).
