The form should start with:  
Before filling out this form, please look at the [guide](https://wiki.mozilla.org/Privacy/How_To/Deidentify) for releasing info about people.

The following fields should be included. All the general questions should be concatenated into the initial bug description.

- Summary [short text field]
- CC [short text field]
- Please attach the data set, or a representative sample: [prompt for attachment]
- Where does this data come from? [long text field]
- What people and things does this data describe, and what fields does it contain? [long text field]
- What parts of this data do you want to release? [long text field]
- Why are we releasing this data, and what do we hope people will do with it? [long text field]
- Is there a particular time by which you would like to release this data? [short text field]

The form should end with:  
Expect to discover that you've missed a couple of things, so plan for a couple weeks to get them corrected. 


Bugs filed through this form should be:

- assigned to [e-mail], with a cc of [e-mail] and [e-mail].
- secret, visible only to the privacy group

Okay, can we change the sentence at the end to:

"Expect to discover that you've missed a few of things, so plan for a couple weeks to get them corrected."

And put it above the submit button?

Once that's done, I think this is good to go live.
