768972 - Add a "remember me" checkbox to the helper apps prompt

We need a way to remember the users choice wrt helper apps.

Probably a "remember" checkbox. This is a bit tricky because our current permissions model requires you to be on the site to reset the permission, which is hard since it will now open in a helper app.

It could be worth filing another bug to try to improve our permissions management UI.
