Launch Firefox. Then navigate to a page containing flash (i.e. youtube.com). The Firefox window loses its focus (title bar color changes to unfocused) and needs to be clicked before accepting input again.

The source of the problem appears to be the Shockwave Flash plugin (version 11.3.300.268 in my case). If it is disabled the bug disappears. Unfortunately to me this is not an option.

Using Spy++ to monitor the window messages in Windows I've discovered the focus loss is caused by the creation of an "Edit" class WS_VISIBLE|WS_POPUP window with zero width and height. My best guess is that the flash plugin creates this window when it displays some flash component.
This is a flash bug and should be reported to Adobe but this already happened, see bug 768802

Workaround: Disable the "Protected mode for Firefox" in flash.
