Steps to reproduce:
I had a favicon for my site, then I bookmarked it. Then I made a new favicon, and then I deleted my bookmark and re-added it to my toolbar. The old icon is used, not the new one. The new one shows in the tab, but the bookmark is using the old favicon.
Actual results:
The old icon is used, not the new one. The new one shows in the tab, but the bookmark is using the old favicon.
Expected results:
The new favicon should be used in the bookmark.

Does this bug also reproduce on older versions of Firefox?

I'm using 15.0.1 -- have seen it in firefox for a long time (years). I just never bothered filing a bug until now.

As far as I know, the favicon is cached. I don't know if clearing the browser cache will help.
If I remember correctly, Marco is the Places owner, maybe he can shine more light here.

After removing the bookmark, try to also remove history for that site (Forget about this site), the icon is cached for 7 days iirc, unless the page is removed from the database, that happens when it has no bookmarks and no history.

This is still an issue in Firefox 55. The comment 4 solution works, but it's unreasonable to perform that action for hundreds or thousands of bookmarks unless you'd like to just flush *all* browsing history even though the history itself is only tangentially related to the icons.
Flushing all history is ultimately the action I took (which worked).
I performed a bunch of actions at the same time, so it's not clear what caused it, but I believe it was related to either refreshing (the release version of) Firefox or loading an old profile before returning to the current one that was exhibiting this problem. (And I only loaded an old profile since refreshing Firefox Dev Edition somehow changed the default profile in the release version of Firefox.)
