As a user, I want to be informed about a subsequent crash on my FirefoxOS phone if I've asked it to always send or to never send a crash report, so that I know why the system rebooted or the app closed suddenly.

On subsequent crashes, display the Crash Banner when the user has asked to "Always send" or to "Never send" a crash. This banner is purely informational, and requires no user action.

Duping to bug 801925, since the work for these two bugs will be the same (modulo adding a "Report" button if the pref isn't set).
