Implement the form error UI from the UX mocks for PIN entry
potch, we probably need to ajaxify the form error handling for this. We can steal from zamboni. Let me or Wraithan know if you need help getting it set up or triggering form errors, etc.

I've prettified the form errors, but we need to do a better job at client-side validation and AJAXing the form submission would speed things up considerably.
