820646 - Project Kickoff Form: New Legal Question to be Added

We'd like another required question added in the legal section if "Vendor/Services" is selected as the "Type of Relationship." 

New question: "Where will the services primarily be provided?" This should have a dropdown list to select from with the following options:

U.S.
Europe
Canada
Global
Another region of the world
A single country (this should have a box to fill in the country)
