Details

I took a picture, and about an hour later I started the camera app again. There is no preview (all black above the camera buttons), and the "take picture" button is unresponsive. I can switch to video mode, but again there is no preview and the button to start the video does nothing. Camera returned to normal after a restart.
OS: Mac OS X → Gonk (Firefox OS)
Hardware: x86 → ARM
Mark: If you are using one of the latest nightlies, I think you are hitting Bug 821810.
I'm on the beta channel, using a build from Dec 12.
I haven't seen this recently, so am going to close as worksforme. If you are still able to reproduce this on a recent phone/build, please reopen.
