To reproduce the bug you must attempt to
task->Privacy and Security->Password Manager->Change Personal Security Password

Immediately after starting the browser.  The result is "Failed to change passwd"
If you cause the browser to prompt you for the PSP (e.g., visit aka.mcom.com -
get a new cert), then you will subsequently be able to change the passwd.

OTOH, if you do

Edit -> Preferences -> Privacy & Security -> Certificate -> Manage Security
Devices -> Software Security Device -> Change Password

It would first prompt you for the password and log in and only then it will
present you with the password change dialog (where you have to type the old
password again). This way everything works, but it is slightly annoying to have
to input the old password twice.

Are the width and height attributes essential in the window.open call?  Fixing
the width and height and not allowing re-sizing of the window makes it hard to
loacalize this window.

We should avoid hard-coding width and height into our xul dialogs.
