Details

The home screen icon should be created before the notification is shown. The icon sometimes takes up to six or seven seconds to be added after the notification has been displayed. I've reproduced this using the Marketplace app at inferno.paas.allizom.org.
Which build/device are you on? Shortcut icons have been reported as not working currently since late March: bug 856131.
I'm on nightly. The shortcut icon for the Marketplace shows up just fine when I install it, but like I said, it takes more than a few seconds *after* the notification has been shown.

P2, but not A4A tracking, given that there isn't a large user impact here.
Synthetic APKs doesn't create a homescreen icon by default, so this is no longer an issue.
