Imitation is the sincerest form of flattery. :) We would like to create a User Engagement Initiation Form just like this one. We need a graphic for the top. Something that looks a little different so people don't visually confuse the forms (another color perhaps?)

I would suggest using the same shape & font for this, but to use a different color. That way we're being consistent, but also differentiating a bit.
