On the new MDN theme, the article TOC items are generated correctly, but they really need some indenting to make the different levels more obvious. See the screen and you'll know exactly what I mean.

https://developer.mozilla.org/en-US/docs/Web/Guide/API/WebRTC/WebRTC_basics is a pretty decent test, as it goes down to h4 level.
