Description:
The "OK" button lights up after the user enters a PUK code even though they have not created a new pin yet in sim security.

Repro Steps:
1)  Updated Buri Build ID: 20131003004003
2)  Tap the "Settings" icon.
3)  Scroll down and tap on "Sim Security".
4)  Toggle "Sim Pin" on.
5)  Enter in an invalid sim pin 3 times so that the sim card becomes locked.
6)  Enter in a valid PUK code and pay attention to the "OK" button.

Actual:
The "OK" button lights up after the user enters in a PUK code.

Expected:
The "OK" button should remain grayed out until the user completes all forms.
