Trying to display a notification with a small icon will render a broken icon image, rather than the actual small icon

1. Go to http://mozilla.github.io/qa-testcase-data/webapi/notifications/
2. Install the hosted app w/permissions
3. Launch it
4. Select show notification with the icon set to smallicon

Expected

A small icon image should render with the notification.

Actual

A broken image icon renders instead with the notification.
