Many of my pages have <html lang="en"> <head> and when I edit them, it remains unchanged as required, but if I enter the same code via the keyboard, it changes. This has been consistent for over a year over a variety of versions.

You don't even need to save it. View in HTML Source. Edit it to have 'lang="en"'. Change the View mode to something else, and then go back to HTML Source, the 'lang="en"' is gone I feel it's not even saved in the attribute list. (though I'm not familiar with Composer code, so I could be wrong).
