Steps to reproduce:
Go to, e.g., http://en.wiktionary.org/wiki/Wiktionary:Main_Page and select 'Add "Wiktionary (en)"' (the Wiktionary's favicon appears just to the left of the text).
Actual results:
Once selected, the search engine/plugin is added to the list, but the favicon doesn't appear; only shows a "empty favicon". (The <os:Image> tag of the proper xml file is lack).
Expected results:
The favicon should appear, but only shows a "empty favicon".

I can confirm that the problem is fixed by increasing MAX_ICON_SIZE
